Nursing

The number one goal of River Valley Home Health & Hospice nurses is to keep our patients at home and out 
of the hospital. Our nurses are responsible for evaluating the patient’s health and environment as well as educating the patient's caregiver 
or other family members on their 
disease process and condition. Nurses will help to coordinate the best care between physician, patient and family, and other care team members. Our nurses help our patients to be more independent and fulfilled through their end of life moments!
Physical Therapy
Home Care & Hospice Care
River Valley physical therapists find it very rewarding as they assist people to reclaim their mobility, independence and daily routines in their own home. The advantages of receiving physical therapy at home are mastering the daily tasks which take place in the home, and to increase safety. It’s in the home setting where most accidents occur and having the PT there allows them to assess home safety, set functional goals, and work with the patient to maximize their ability, improve strength, balance, coordination, and make strides by enhancing their quality of life as their physical impairment declines and mobility increases. For improved ADL (Activities of Daily Living) function, they will demonstrate and teach safe ambulation and transfer techniques with or without assistive devices (e.g., walker, getting out of bed, up from sitting, maneuvering safely with a walker or wheelchair) and develop a home exercise program with you to maintain muscle tone and mobility.
Physical therapy (PT) is care that aims to ease pain and help you function, move, and live better.People of all ages get physical therapy. It can treat a variety of health problems.Our licensed health professionals get specific training in physical therapy. You may hear them called PTs or physiotherapists.

Medical Social Work (Home Care & Hospice Care)

River Valley Medical Social Workers evaluate and assist patients and families to help them adjust to the social and emotional factors related to their health challenges in their place of residence. MSWs make assessments and work to understand each patient’s personal goals and most pressing needs. They help patients and their families cope with issues by connecting them with community resources, educating on options, and facilitating discussions between the patient and their family.
Medical Social Worker Services may include:
Information regarding community resources, financial assistance, fiduciary services, and power of attorney services, among others.
Financial consultations including the application for financial aid. 
Assisting with identifying long-term care needs and placement, if necessary.
Speech Therapy
Home Care & Hospice Care
River Valley’s speech/language pathologists provide in-home assessment and interventions for conditions such as Parkinson’s disease, stroke, ALS, head injury, dementia, COPD and CHF or anything that results in an individual losing the ability to form words and understand language or developing speech disorders or a difficulty in swallowing. This can seriously hinder that individual’s ability to continue living independently in a safe and healthy manner. Our STs can help the patient to participate in more complex word games and activities that enhance speech retrieval. As progress or setbacks are noted, each activity is carefully evaluated for effectiveness, and activities can be changed, increased or eliminated at any time in order to effectively work toward the client’s personal and therapeutic goals. River Valley’s understands the importance of family support when speech therapy is prescribed, and that is why our team of professionals includes family members in the rehabilitative process. The patient, caregiver and their family are encouraged to ask questions, make comments and receive appropriate training from our speech therapists. Family is also included when the plan of care is developed, updated or reviewed. River Valley offers an inclusive form of therapy that is both unique and effective.

Home Health Aides & Hospice Aides

River Valley Home Health Aides provide health care tasks with a team approach working with OT, PT and nurses, by assisting patients in personal services such as bathing, dressing, and grooming. HHAs can help by observing a patient’s physical and mental condition, intake and output, and exercise.

Dieticians (Home Care & Hospice Care)

River Valley Registered Dieticians, organize, and conduct dietary education for agency personnel. They function as a member of the home care team with expertise in nutritional counseling by instructing care team and patients on nutritional principles, dietary plans, food selection and economics.
